What is social prescribing?
What is a Lifestyle Advisor?
It’s estimated that one in five of the people who go to see their GP are troubled by things that can’t be cured by medical treatment. GPs tell us that they spend significant amounts of time dealing with the effects of poor housing, debt, stress and loneliness.
Many people are overwhelmed and can’t reach out to make the connections that could make a difference to their situation. This is especially true for people who have long-term conditions, who need support with their mental health, who are lonely or isolated, or who have complex social needs which affect their wellbeing.
Our social prescribing link workers help people to identify what matters to them, and work out how to connect with the activities that might make a difference.
Social Prescribing is suitable for any patient whose health & wellbeing is being impacted by practical or social issues, such as:

A Lifestyle Advisor is here to help you get your lifestyle back on track.
If you’ve put on some extra, unwanted weight and want to get your body back to being fit and healthy, or you’ve got a bad habit such as smoking or drinking, a Lifestyle Advisor is able to assist you in making the best changes for you.
Lifestyle Advisors are able to guide you and provide you with the best tools and advice to help you make the most positive changes to your wellbeing and health.
A Lifestyle Advisor is suitable for any patient who wants to feel healthier and happier, by helping with:
Making healthier food and drink choices
Stopping smoking
Reducing alcohol intake
Becoming more physically active, which really can be as easy as a walk in the park!
Overall, these changes will help you to improve your wellbeing and keep your body healthy, for a longer and happier life.
How to access this service.
We’ve partnered with Onside Advocacy, who provide Social Prescribing services across Worcester City Primary Care Network.
Each practice has links with a Wellbeing Team, provided by Onside. You can be referred to them by your GP, by completing the form below.
